ECG Blog #116 (ECG Video-Blog-13) – Essentials of Axis/Hemiblocks

This is the 13th installment of my ECG Video Blog. This 42-minute video covers the Essentials of Axis Determination and Assessment of the Hemiblocks. I begin with the basics – so the material is understandable for any level of interpreter. There follows a gradual progression (with PEARLS for all level providers along the way) that should leave you comfortable with an easy-to-remember and accurate method for determining both Axis & Hemiblocks in less than 5 seconds.

           My goal in this 42-minute video series is to present a user-friendly approach that allows determination of Axis and the presence of LAHB within less than 5 seconds. LPHB is far less common than LAHB. It rarely occurs as an isolated defect — but instead may occasionally be seen in association with RBBB. 
      • LPHB can be recognized by the finding in "B" in Figure-1 below — in which there is a very steep initial descent to the S wave in lead I.
        Figure-1: Schematic illustration of the 2 types of Hemiblock that may be seen in association with RBBB. The findings in "A" — are consistent with RBBB/LAHB (predominantly negative QRS in lead II). In contrast — "B" is consistent with RBBB/LPHB (very steep straight initial component to the S wave in lead I). NOTE — Enlarge by clicking on Figures — Right-Click to open in a separate window.
